What someone truly feels or wishes. And it's presence reaches far. So when Rukia came to earth the gem stuff basically granted all their wishes or rather their heart desires. Chard wanted to protect so he got his arm, Ichigo wanted to save his family so he became a shinigami and Orihime ....well you get the point.
